We offer a warm and caring environment where you’ll get devoted time with the physician. They will listen to your concerns and help you select the best treatment option. Please ask questions and feel free to bring a family member or loved one. Some patients find it helpful to write down questions ahead of time. You may want to take notes of some items discussed during the visit.
Please be on time for your appointment to ensure that we are able to devote the appropriate amount of time to your care without being rushed. If you aren’t able to fill out new patient paperwork prior to your visit (from the website), please arrive 15 minutes prior to your appointment time to do so. Bring insurance card(s), picture ID, referral letter (if required by insurance) and your copay. If you have FMLA forms to be filled out, bring them to your appointment or fax to 817-527-6610.
Additionally, so that we can better understand your full medical history, please bring your medication list and medical records (if referring physician’s office doesn’t send). Breast patients should also bring films/reports of all imaging and biopsies done.

We will gladly bill your insurance for our services at the contracted rate. Baylor Scott & White Surgical Group of North Texas fees are standard for the specialties we offer and the region in which we are located. Information regarding these fees is available upon request. Please be aware that the patient is ultimately financially responsible for all services provided. We will bill your insurance carrier; however, any fees not incurred by your insurance carrier will be the responsibility of the patient.
You deserve to understand your insurance policy coverage and your carrier’s customer service department should be able to provide a clear explanation.
